In detail

How the studies gave it

  • A standardized water extract of root, made to .8 to 1.5% eurycomanone. Root powder and other extracts are not comparable.
  • 50 to 400mg a day, most often 200mg. The research stops at 400mg, so more is not better.
  • Once a day, every day. Time of day, and taking it with or without food, were not compared in the studies.
  • Daily for up to 6 months, without breaks. Six months is the longest study so far.
Results

How the studies measured change

Blood testsHormonesTotal testosterone, and in some studies cortisol, before and after. Usually taken in the morning, when levels are highest.
QuestionnairesHow people feltFatigue, mood, aging male symptoms, sexual function and menopause symptoms, on published scales.
Strength testsPerformanceLeg and grip strength, in people who were also training.

If you and your doctor decide to track your own response, the same measures, taken the same way before and after, make the comparison fair.

Which product

Choosing a Tongkat Ali product

  • Choose the extract that was studiedThe human studies used a standardized water extract of root, .8 to 1.5% eurycomanone, most often at 200mg a day. Physta®, sold in North America as LJ100®, is the extract tested in most of them, and the most clinically researched Tongkat Ali extract.
  • Look past the ratioA ratio such as 200:1 says how much root went in, not what came out. In laboratory tests of 22 extracts sold as Tongkat Ali, two met Malaysia’s national standard. What the testing found
  • Root powder is a different productDried, ground root is far less concentrated than an extract, and it is not the form tested in the human studies.
  • Read the labelLook for milligrams per serving, a eurycomanone percentage, and the words “water extract” and “root”. Compare a label with the research

Common questions

Is this a protocol to follow?

It is a summary of what published studies did. Whether Tongkat Ali suits you, and how it fits with any treatment you take, is a question for your doctor.

Does it need to be taken with exercise?

Not in the testosterone, energy or stress studies. The strength studies combined it with training.

Is a higher dose stronger?

Doses above 400mg a day, and extracts above 1.5% eurycomanone, have not been studied in people.

Is Tongkat Ali an alternative to TRT?

They do different jobs. TRT is a prescription treatment for diagnosed low testosterone, managed by a doctor. Tongkat Ali is a supplement studied for supporting the body’s own testosterone.

What if I notice side effects, or nothing changes?

Stop and speak to a doctor about anything unexpected. Fatigue, low mood or low drive that continue can have causes worth finding, from thyroid problems to sleep apnea.
